This form is to be filled out in order to be accepted for Legacy Sponsored Childcare at the YMCA. In December of 2023, Legacy Church gave $10,000 to the YMCA as a grant for Legacy Church attendees to get discounted childcare through the YMCA and Legacy Church partnership.
